To perform the DNA analysis and create the individual analysis report, we require additional information about the person who will take the test.
Please fill out the registration form below with your order number (in the subject) as well as name, contact details, age, height, and weight of the corresponding person.
Have you ordered several DNA tests for multiple people?
Then please list additional people subsequently with their names, contact details, age, height, and weight.
You have ordered different DNA tests ("My Health & Power" and "My Health" & "My Beauty") for different individuals?
Please also specify which test will be performed by which person.
Please note: The DNA test can only be shipped after registration is complete!
